Griswold, Ceresti Link Services to Expand Dementia Care Access

BLUE BELL, PA — Home care provider Griswold and dementia care company Ceresti Health have partnered to combine in-home caregiving services with Medicare-supported dementia management programs, a move aimed at addressing growing demand for coordinated care as the number of Americans living with dementia continues to rise.

The partnership will connect eligible families enrolled in Ceresti’s dementia care program with Griswold’s nationwide network of caregivers, providing in-home respite and support services for both dementia patients and family caregivers.

The initiative is tied to the Centers for Medicare & Medicaid Services’ Guiding an Improved Dementia Experience, or GUIDE, Model, a federal program designed to improve care coordination and expand support services for people living with dementia and those caring for them.

Under the arrangement, Ceresti will provide dementia care management and caregiver support services, while Griswold will deliver in-home care and respite assistance through its local offices.

The collaboration reflects broader efforts across healthcare and home-care industries to address gaps in dementia care, particularly outside of clinical settings where family members often shoulder much of the responsibility for day-to-day care.

According to CMS, the GUIDE Model seeks to reduce caregiver burden, improve quality of life for patients, and help individuals with dementia remain in their homes longer by expanding access to coordinated services.

Matt Ericksen, Griswold’s vice president of sales and operations, said the partnership combines Medicare-supported dementia resources with the company’s existing home-care services.

“Partnering with Ceresti strengthens our ability to support clients and their loved ones with Medicare-supported dementia navigation, education and respite hours,” Ericksen said.

Ceresti Chief Executive Officer Dirk Soenksen described the partnership as an effort to address challenges families face between physician visits and throughout daily caregiving responsibilities.

“Dementia care breaks down when families are left to navigate complex needs alone, especially between physician visits and in the day-to-day realities of the home,” Soenksen said.

The agreement comes as healthcare providers, insurers, and policymakers increasingly focus on home-based care models as a way to improve outcomes and potentially reduce costly hospitalizations and institutional care for people with chronic conditions.

Griswold provides non-medical home care services through a nationwide network, while Ceresti specializes in dementia management programs that support patients and caregivers through education, care coordination, and ongoing monitoring.
