WEST CHESTER, PA — The Chester County Health Department cited two food establishments for violations during inspections conducted May 4, 2026, including sanitation issues, equipment deficiencies, and food safety risks, and advised residents to report concerns online or by calling 610-344-6225 if illness occurs after dining.
At Exton Senior Living, 600 N. Pottstown Pike in Exton, inspectors cited multiple violations related to facility maintenance and cleanliness.
Issues included frost and ice accumulation around a walk-in freezer door, a broken light shield near the exhaust hood and non-functioning or dim lighting in kitchen and restroom areas.
Inspectors also cited the facility for unclean equipment and surfaces, including shelving in refrigeration units, a convection oven, a flat-top griddle and an Alto-Shaam unit, along with wall surfaces near the exhaust hood and dishwashing area.
Additional violations included damaged wall and door frame surfaces near the ice machine that were not smooth or easily cleanable.
Most violations were required to be corrected within seven to 10 days, and a staff member was instructed to apply for food safety certification reciprocity within seven days.
At Fysh Sushi and Poke, 5 S. Morehall Road in Malvern, inspectors cited food safety violations involving improper storage, sanitation and handling practices.
Inspectors observed raw fish and chicken stored above ready-to-eat foods, creating a cross-contamination risk, and noted the use of reduced oxygen packaging for raw fish without an approved Hazard Analysis and Critical Control Points plan, requiring removal of a vacuum sealer until compliance is achieved.
Additional violations included an obstructed handwashing sink, an open employee beverage near food preparation areas, improper thawing of vacuum-sealed fish and a missing light bulb in the hood system.
Inspectors also cited improper cleaning practices, including washing food-contact equipment in a prep sink instead of a designated sanitizing station, and identified grease buildup under a grill and improper mop storage.
Several violations were corrected during the inspection, including relocation of raw foods, removal of obstructions from hand sinks and proper labeling of containers.
