WEST CHESTER, PA — The Chester County Health Department reported multiple food safety violations at three licensed food facilities following inspections conducted Sunday, April 27, according to official inspection records.
The violations were identified at Brothers Pizza & Grill LLC in Malvern, Kadima Rehab & Nursing in Pottstown, and Roots Cafe in West Chester.
At Brothers Pizza & Grill, inspectors cited issues including improper food temperatures, missing labels on food containers, and equipment concerns.
Rice and beans were observed held at 115–120°F, below the required 135°F for hot foods, increasing the risk of bacterial growth.
Inspectors also reported raw meats, cheeses, and other temperature-controlled foods stored at 45°F in a walk-in refrigerator, exceeding the required maximum of 41°F.
According to the report, food items were discarded during the inspection, and the refrigerator must be repaired before further use.
Additional violations included broken floor tiles in the walk-in fridge, lack of a probe thermometer, improper dishwashing setup, and buildup of residue inside an ice machine.
At Kadima Rehab & Nursing, inspectors cited a lack of food safety knowledge by the person in charge, along with sanitation and maintenance issues.
Violations included an unclean can opener blade, missing paper towels at a handwashing station, and improper food dating practices.
The facility was also cited for having a certified food manager listed who is no longer employed, with officials requiring proof within 10 business days that a staff member is enrolled in certification training.
At Roots Cafe in West Chester, a follow-up inspection found continued violations related to food temperature control and facility maintenance.
Inspectors reported prepared foods stored between 49–51°F instead of the required 41°F or below.
The affected items were relocated during the inspection, and the unit must not be used until it can maintain proper temperatures.
Additional issues included missing allergen warnings on menus, unclean food contact surfaces, deteriorating floor and ceiling areas, and gaps in an exterior door that could allow pests to enter.
According to inspection notes, some violations were corrected on site, including cleaning of equipment and relocation of food items.
The Health Department noted that inspection results reflect conditions observed at the time of inspection and may not represent long-term compliance.
