AUDUBON, PA — The Lincoln Center for Family and Youth and Greenspace Health are launching a state-funded digital maternal health platform in Montgomery and Lancaster counties to connect pregnant and postpartum individuals with mental health care, case management and community services through a single system.
The two-year initiative, called Bloom, is funded by the Pennsylvania Department of Health’s Bureau of Family Health. The organizations plan to expand the platform to Wyoming and Sullivan counties during the project period.
Bloom combines Greenspace’s population-health technology with measurement-based care, which uses patient-reported information to assess needs and guide care. Users will receive recommendations based on factors including their reported challenges, demographics and location.
The platform is intended to address gaps that can prevent pregnant and postpartum individuals from obtaining mental health and other services, including cost, transportation, stigma and difficulty navigating available resources.
“Too many mothers, particularly those navigating poverty, limited transportation, or a lack of culturally responsive care, reach out for help and are met with significant barriers,” said Emily Gatto, senior vice president of TLC Wellness at The Lincoln Center for Family and Youth.
Bloom will be free and confidential and will connect users with maternal health resources, community referrals and dedicated case-management support.
The initial rollout focuses on Montgomery and Lancaster counties. The organizations cited Pennsylvania Department of Health data showing disparities in preterm births, low birth weights and access to first-trimester prenatal care among populations in the targeted communities.
The project also seeks to address low rates of treatment for maternal mental health conditions. Research cited by the organizations found that only 15% of pregnant people experiencing depression and anxiety are connected with care.
Greenspace’s technology will collect user-reported outcome measures that can be used to tailor recommended support. The company reported a 43% 30-day retention rate and average symptom improvement of 19% across its existing Population Health Hubs.
The Lincoln Center for Family and Youth is an Audubon-based nonprofit providing community mental health services. Greenspace Health provides measurement-based care technology for behavioral health organizations.
