EXTON, PA — Exton Post Acute, a recently rebranded skilled nursing facility in Chester County, has launched a new Chronic Illness Support model aimed at delivering comprehensive, patient-centered care for individuals managing complex medical conditions. The initiative is being implemented in collaboration with Constellation Health Services and is designed to integrate palliative care, pastoral support, and other holistic services into the facility’s specialized clinical programs.
The Chronic Illness Support model is available to clients of Marquis Health Consulting Services, which supports the facility. According to Erin Popelak, regional director of market development at Marquis, the program represents a “next-level model of care” and underscores the importance of addressing both physical and spiritual needs. “Exton’s new program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in Constellation Health Services,” said Popelak.
At the core of the initiative is a robust palliative care offering, administered by a dedicated team of physicians, nurses, and palliative care specialists. This component is designed to manage symptoms and reduce the emotional and physical burdens associated with chronic conditions, while supporting patients’ overall quality of life.
Leading the program for Constellation Health Services is Christen Hall, DNP, CRNP, a board-certified nurse practitioner with advanced expertise in palliative care, public health, and epidemiology. Hall’s leadership is expected to bring a high level of clinical acumen and patient advocacy to the program.
The Chronic Illness Support model also enhances Exton Post Acute’s existing specialty care services, including Cardiac Care and Pulmonary Care. Additional elements include disease-specific education, coordinated care from multidisciplinary specialists, social services, health coaching, and access to community-based support and enrichment activities.
Located at 501 Thomas Jones Way in Exton, the center provides post-acute, short-term rehabilitation, and long-term skilled nursing care. The new programming signals an expanded commitment to comprehensive, compassionate care for individuals facing the challenges of chronic illness.
