According to police, the incident occurred on Friday, November 7, 2025, at the juvenile facility located at 1000 Academy Drive in New Morgan Borough, Berks County. Investigators report that the attempt began when a 15-year-old identified as J.C. allegedly kicked open a door leading to the outside grounds. Two others—17-year-old Y.P.C. and 18-year-old Diandy Jaoquin-Alfonseca—reportedly followed him into the yard.

Surveillance footage reviewed by police shows the three individuals running across the grounds and attempting to climb a perimeter fence. Facility staff and guards responded immediately. Officials emphasize that all charges are allegations, and any suspect, arrestee, or defendant is presumed innocent until proven guilty in a court of law.
